AUBAGIO One to One Start Form

Eligibility

Patients must be prescribed AUBAGIO for multiple sclerosis and sign the required authorizations for support services and health information disclosure. The One Start program provides temporary no-cost therapy for commercially insured patients but excludes those with government insurance like Medicare or Medicaid.
