PAN Foundation Chronic Kidney Disease (CKD) Medication Copay Assistance

Eligibility

Applicants must be receiving treatment for chronic kidney disease in the United States, have government-insured health coverage (Medicare, Medicaid, or TRICARE), and fall at or below 400% of the Federal Poverty Level. The grant specifically covers out-of-pocket costs for medications on the PAN Foundation's approved list for CKD.
