Eligibility
Eligible patients must be US or Puerto Rico residents aged 18 or older with commercial health insurance prescribed VALCHLOR for Stage IA or IB mycosis fungoides–type cutaneous T-cell lymphoma. Individuals enrolled in government-funded programs like Medicare, Medicaid, or Tricare are excluded from the copay assistance program.
